The National Library of Medicine style guide, Citing Medicine (Second Edition) is now available online. Free access to the revised edition includes recently added information about citing Internet resources (blogs, wikis, multiple versions of online journal articles) as well as old school resources (audio- and video-cassettes, slides, photographs, CD-ROM, DVD and print).
